You qualify if you are a California resident who received direct mail notice from Total Vision about the October 30, 2020 data security incident.

fund was stored as 47500000 -- 100x the real $475,000 GSA -- fixed. payout_text was truncated mid-word ("out-of-pock") -- fixed. The claim-form subpage footer states a deadline of September 21, 2026, but every other mention on the site (top banner, FAQ, key dates section) says October 5, 2026. Kept October 5 since it is the majority-supported date across the site, but worth a manual check against the Long Form Notice PDF if this comes up.
