A settlement has been reached with Lakeview Health Systems, L.L.C. ("Lakeview Health" or "Defendant") in a class action lawsuit. This case is about the targeted cyberattack on Lakeview Health's computer systems that occurred in January 2024 (the "Data Incident"). Certain files that contained private information were accessed. These files may have contained personal information such as names; addresses; dates of birth; Social Security numbers; driver's license numbers; financial account numbers; patient IDs; medical treatment information; diagnoses; conditions; prescription information; and health insurance information. Settlement benefits: Ordinary Losses (out-of-pocket expenses): up to $2,000.00 with documentation; Extraordinary Losses (losses from identity theft or fraud): up to $5,000.00 with documentation; Lost Time: up to four hours, at $20.00 per hour, for a maximum of $80.00; Alternative Cash Payment: expected to be $50.00 (no proof required); Credit Monitoring: one year of CyEx Medical Shield Total with $1 million of medical identity theft insurance. The only way to receive benefits or payments from this Settlement is by submitting a valid and timely Claim Form. ... If you do nothing, you will not receive a benefit from this Settlement. You will also give up the rights described in Question 8.
Who may qualify
All individuals residing in the United States whose PII/PHI was compromised in the Data Incident discovered by Lakeview Health, on or around January 25, 2024.
