Unauthorized access to Henderson & Walton Women's Center's computer systems in February 2022 exposed patients' personal and health information. Affected individuals can claim up to $150 for ordinary losses, up to $2,500 for extraordinary losses, or up to $90 for lost time.

Who may qualify

Eligible if: All persons whose personal information and/or protected health information was potentially compromised as a result of the Cybersecurity Incident which occurred between February 11, 2022, and February 14, 2022.
